To claim the self-employed health insurance deduction, verify that your plan qualifies, separate eligible premiums from reimbursements, and calculate the limit tied to your business income. This guide covers 2025 returns filed in 2026 and the core rules for 2026 planning.
You may qualify if you have a net profit from self-employment, qualifying partnership income, or qualifying wages as a shareholder owning more than 2% of an S corporation. Coverage must be established under the business. A sole proprietor’s policy may be in the individual’s name or the business name. Partnership and S corporation arrangements require additional payment or reimbursement steps.
You cannot claim this deduction for months when you were eligible for a subsidized employer health plan through your own employer, your spouse’s employer, or an eligible dependent’s or under-27 child’s employer. Choosing not to enroll does not remove this restriction. See the complete eligibility guide before calculating your deduction.
Collect health, dental, and qualifying long-term care insurance statements for yourself, your spouse, dependents, and a child under age 27 at year end even if that child is not your dependent. Long-term care premiums have annual limits based on age. Exclude pretax amounts, reimbursements, and amounts receiving another tax benefit.
Do not assume former-employer coverage qualifies. Our COBRA premium deduction guide explains that distinction.
The deduction cannot exceed the earned income available from the business under which the plan is established. For a sole proprietor, this calculation generally accounts for the deductible part of self-employment tax and deductible retirement contributions. Net Schedule C profit alone is not always the final limit.
For illustration, if your properly calculated limit is $8,000 and eligible premiums are $10,000, the self-employed deduction cannot exceed $8,000. The remaining eligible costs may be considered under Schedule A medical expense rules, without claiming the same dollars twice.
For 2025, many taxpayers can use the worksheet in the Form 1040 instructions. Form 7206 is required when you have more than one source of income subject to self-employment tax, file Form 2555, or include qualified long-term care premiums. Consult the Form 7206 instructions for your circumstances and use the tax-year version of the form.
If your policy came through the Marketplace, the premium tax credit and self-employed deduction may affect each other. Simply subtracting an initial credit estimate is not always sufficient. Use the calculation procedures in IRS Publication 974 or software that handles this coordination. Your Form 1095-A supplies important monthly amounts.
The deduction is reported on Schedule 1 with your individual return. You can claim it without itemizing. It reduces income for federal income tax purposes, but does not reduce net earnings used to calculate self-employment tax. Keep policies, receipts, reimbursements, and your calculation with the return.
If you need more filing time, request a personal tax extension by the applicable deadline and pay estimated tax on time.
A timely tax extension gives you additional time to file your return. Submit your request by the applicable deadline, and pay any tax due by the original payment deadline. An extension to file is not an extension to pay.
